[chore] small changes missed in previous dereferencer.GetAccount() PRs (#1467)
* small formatting changes, rewrite fetchRemoteMedia to use separate funcs + use mutex lock correctly * move url parsing before acquiring mutex locks * use wrapped mutexes to allow safe unlocking.
